If you are unable to keep your appointment, we ask that you kindly provide us 24 hours notice. This courtesy, on your part, will make it possible to give your appointment to another patient. If we are not provided 24 hours notice, there is a $150 cancellation fee for medical appointments and a $200 cancellation fee for cosmetic appointments.

Patient Portal - For Dr. Glodny's Patients
Upon scheduling your initial appointment, you will receive an email verification to your preferred e-mail to access the Park Avenue Dermatology patient portal via Sadio.com. Please fill out your medical history online before you arrive for your appointment if possible. Also, please arrive 10 minutes prior to your appointment time to allow for the check-in process.
Items to bring to your appointment are:
- Valid Photo ID
- Insurance Cards, both Primary and Secondary
- Valid Credit Card/HSA Card
- List of Current Medications
Patient Forms- For Dr. Warner's Patients
The following forms can be downloaded to your computer and printed at home. By filling out these forms ahead of time, you will save a significant amount of time during your visit.
During this office visit Dr. Glodny or Dr. Warner will evaluate any and all of your cosmetic concerns. They will provide a thorough treatment plan and will take the time to answer any and all of your questions. The charge for this visit is $350 which will go towards the cost of your procedure. Your procedure may be performed the same day as your visit or at any time over the following 3 months.
